Students are welcomed to share their own experiences from their respective home countries on ways of fostering continuity of care.

This is an optional course for cohort 20. For future cohorts it is a mandatory course in the Major HSR.
Umfang Blockveranstaltung
Inhalt o Aspects of continuity of care: Management continuity, informational continuity, contact and relational continuity
o Different approaches to enhance continuity of care at all level of the health systems: continuity in care for individual, continuity within and across health services and programs, as well as cross-sectoral care
o Relevant health strategies across the continuity of care
o Quality management and outcome measurement across the continuity of care
E-Learning Teaching material will be provided via the e-learning platform Moodle.
Lernziele Students are able to
- delineate key aspects of and different approaches to ensure continuity of care in particular for people with chronic health conditions.
- critically evaluate existing programs and services tailored toward continuity of care with reference to its key aspects.
- argue for the relevance of continuity of care in response to a specific challenge in health services provision.
- conceptualize exemplary services and programs tailored toward the needs of a specific group of individuals that reflect relevant aspects of continuity of care.
