This is an optional course for cohort 20. For future cohorts it is a mandatory course in the Major HSR. |

o Aspects of continuity of care: Management continuity, informational continuity, contact and relational continuity
o Different approaches to enhance continuity of care at all level of the health systems: continuity in care for individual, continuity within and across health services and programs, as well as cross-sectoral care
o Relevant health strategies across the continuity of care
o Quality management and outcome measurement across the continuity of care |

| Lernziele |
Students are able to
- delineate key aspects of and different approaches to ensure continuity of care in particular for people with chronic health conditions.
- critically evaluate existing programs and services tailored toward continuity of care with reference to its key aspects.
- argue for the relevance of continuity of care in response to a specific challenge in health services provision.
- conceptualize exemplary services and programs tailored toward the needs of a specific group of individuals that reflect relevant aspects of continuity of care. |
