Students are welcomed to share their own experiences from their respective home countries on ways of fostering continuity of care.
Umfang Blockveranstaltung
Inhalt
o Aspects of continuity of care: Management continuity, informational continuity, contact and relational continuity
o Relevant health strategies along the continuum of care
o Different approaches to enhance continuity of care, such as integrated or managed care
o Institutional ethnography as a method of inquiry 
- to delineate structures and processes inherent to the care of people with chronic health conditions
- to reveal steering mechanisms underlying these structures and processes
- to critically reflect how well these structures and processes respond to the needs of individuals
E-Learning Teaching material will be provided via the e-learning platform Moodle.
Lernziele Students are able to
- delineate key aspects of and different approaches to ensure continuity of care in particular for people with chronic health conditions.
- critically evaluate existing programs and services tailored toward continuity of care with reference to its key aspects as well as to what extend they are targeted toward the needs of a specific group of individuals
- argue for the relevance of continuity of care in response to the needs of persons with chronic health conditions as well as specific challenges in health services provision for the respective group of persons
- apply methods relevant to Institutional Ethnography to describe how health care programs are targeted toward the needs of persons with chronic health conditions
